Have all of your immunization shots already? Don't wait.
Many of you still haven't submitted your immunization records. The Academic Advising Office cannot register you for courses until the Student health Service (SHS) has received documented proof of your immunizations. If you have all of your immunizations (call the doctor's office to find out), you'll just only need to drop off your Medical History form at the doctor's office for your doctor to fill out the immunization section and pick up within a few days to mail to SHS or bring a self-addressed and self-stamped envelope and have the form sent directly to SHS.
An appointment with your doctor is not necessary unless you are missing some immunization shots. Some students have said they are unable to schedule an appointment until late in the summer and are, therefore, unable to send the form back. If you still want a physical exam and have all of your immunizations already, you can still have your doctor's office fill out the immunization section regardless and mail the Medical History form back to Gallaudet prior to your physical exam appointment.
